First Steps
Referrals and requests for more information come from patients, families, friends, health care professionals, and spiritual counselors. It is an easy three step process to make a referral of Hospice Compassus.
Step One:
Place the call to Hospice Compassus.
Refer to the ‘Locations’ page for the hospice nearest you.
Step Two:
Our Admissions Coordinator will contact all responsible parties to coordinate family consultations and/or patient assessments.
Step Three:
Our Admission Team consult, evaluates and initiates care, if appropriate.
